We collect personal information when you commence any of our services with us. This may include personal details such as name, address and date of birth, but is not limited to additional information as your training history, health history and medical history. We collect and utilise this information for provision of services to you and we protect this information in line with the Privacy Act 1988. This information is collected with the primary purpose of providing quality health care, nutritional advice and fitness programming to you. We only collect the personal information or additional information as deemed necessary to provide the goods and services you require. Personal information may refer to any information by which your identity is known or can be reasonably ascertained. Additional information may refer to medical history, injuries and conditions so that we may proactively assess and conduct our duty of care in order to treat and train you in the safest way possible. You are not obliged to provide any information requested, however failure to provide this information may compromise the quality of your health and fitness management.
When making a booking online or otherwise with Healthy Society, we require your personal information to ensure your information and subsequent services are managed with the utmost confidentiality and privacy. Pre-consultation forms will be provided for health consultations, you are not obliged to provide information on these forms, however we suggest thorough completion of these forms to make your consultations more efficient in discovering the most effective treatment strategy for you. If you do provide health information in this manner, you do so with acknowledgement of the risks of third-party interference on our website and email servers. Despite our upkeep of SSL certificates and security on our interfaces, third-party interference is outside of our control.
The personal information held about you in consultation notes, test results and training programs is either provided by you or as consequence of information provided by you during provision of services to you. This information is held in accordance with Privacy policy and are by law also your personal information and so are available to you upon request and verification of your identity.

Use of your information Healthy Society uses your information primarily for the provision of services to you. We may also use your information to include on our mailing list regarding updates, education, services and motivation which may be of interest to you along with communications regarding appointments and the provision of service to you. Disclosure of your information During the time you receive health or fitness management or care from Healthy Society, we may disclose your information:• to facilitate a referral to another practitioner• to communicate with one of your other health practitioners for effective referral• communication between Healthy Society practitioners and coaches to ensure safe and effective health and fitness management to you• in the event of medical emergency where we will contact your nominated emergency contact, and appropriate third parties; and• to facilitate any other management of your health care as requested by you.
We may engage external professional services to assist us with our business developments. These third parties may have access to our information to perform their function, but they too are bound by privacy policy and cannot use that information for other purposes. We will not otherwise disclose your information without your consent, unless we are required to by law. Security We only keep such information as required by law and that is necessary to continue the provision of service to you in a safe and ethical manner.
